Home Health Care RN- wellness nurse for a TalentBurst Private client. Provide warm and engaging personal care services, assistance, support, and companionship. Administer/monitor medication for the patient based on the prescribed care plan. Engage with the patient while checking on their well-being and communicate changes in condition or care plan. Monitor Blood Pressure and assist with mobile movement and meal plans. Collaborate with the patient's family and health care team in assessing and documenting resident assessments at set intervals. Must be BLS/ACLS certified, NIH (Stroke certification), Licensed Practical Nurse (LPN/LVN) or Registered Nurse preferred. Client requested a visit 3 times a day, 8 am-Noon-5 pm to start. 12-week assignment to start. Location: Fort Collins CO.
