Licensed Practical Nurses (LPNs) provide skilled nursing care to patients in their homes, following physician orders and care plans. They perform hands-on medical tasks such as wound care, catheter management, and patient assessment, while educating patients and families to improve health outcomes. LPNs work with interdisciplinary teams to deliver personalized home health services mainly for individuals under Medicare, Medicaid, and private insurance programs.

Position Overview
The Licensed Practical Nurse is an essential part of the team responsible for providing and documenting skilled nursing care in accordance with the developed care plan and physicians’ orders for each individual patient while adhering to confidentiality standards and professional boundaries at all times.
